All Students/Learners (Main Campus & Professional Studies)

Application for Degree - This form is to be completed in the term prior to completing your degree, minor, and/or Certificate. This must be completed to graduate. Please note the deadlines. 

Duplicate Diploma Request - This form is used to request a duplicate diploma.

Name Change Form - This form is used to change your legal name in the system.

Non Disclosure Form (FERPA) - This form is used if you wish to not have directory information available in the Student Information System.

Student Consent Form (FERPA) - This form is used to allow a student to give consent and access to academic records to others outside of the student.

Transcript Request Form - This form is used to request a paper transcript to be sent via mail.

Incomplete Request Form - This form is used to request an incomplete in a course.

Incomplete Extension Form - This form is used to request an extension to an incomplete.


Main Campus Students

Academic Suspension Appeal Form - This form is to be completed to appeal for readmission following academic suspension.

Add Drop Form - This form is used to request schedule changes that cannot be completed on self-service.

Course Withdrawal Form - This form is used to withdraw from a course after the add/drop deadline has passed. 

Declaration / Change of Major / Minor / Emphasis - This form is used to declare and/or change your major, minor, or emphasis.

Withdrawal from Southwestern College - This form is used if you do not plan to return to Southwestern next term or you are withdrawing immediately. 

Study Abroad Form - This form is to be completed if you intended to study away from Southwestern College.

Unscheduled Course Request - This form is to be completed for internships, independent study, field experience.

Request for Substitution of Course (MC) - This form must be completed to request a substitute for a required course at Southwestern College.


Professional Studies Learners

Academic Suspension Appeal Form - This form is to be completed to appeal for readmission following academic suspension.
