Student Organization Application for Recognition
Any student organization wishing to be recognized by SGA for the academic year must complete the Student Organization Application for Recognition and the Hazing Awareness Agreement as well as have the Approval Request submitted by their faculty/staff advisor.

Budget Proposal Request
Funding Usage Request
This form is for organizations that need to request funding usage for various SGA-related organization events or activities.
Please fill out the form completely. All approvals will be sent to sga@sckans.edu. Please contact SGA if you have any questions.
Requests take 3-5 business days to get sent to the business office. SGA will contact you and your organization when the funds are ready for use.
Event Approval Form
Any SGA recognized organization must complete an event approval form to the Campus Life Office for approval of any events the organization wishes to hold on or off-campus. Please read the event approval guidelines carefully, then submit the event approval form to the Office of Campus Life for approval of any event happening through your SGA-recognized organization.

Fundraising Profit Report Form
The fundraising should be completed to report fundraising profits .
